Information Sharing

When the time comes your child’s other educational settings may require information on your child. We feel it is important for your child that their key workers has as much information as possible. If you would like any information shared please sign below. I would like any information on my child shared with their other settings *

Medical Treatment

I give permission for a plaster to be administered to my child’s skin should the need should arise. *

I understand that should my child require emergency medical advice or treatment in the event of an accident within nursery, Growing Stars Nursery staff will do everything possible to contact me. Should this not be possible, I give consent to the seeking of any emergency medical advice or treatment. *

Child Protection Statement

The children are our main priority and as such we have a duty to report suspected child abuse and neglect. If we have concerns we will follow the guidance issued by the Local Authority of Harrow Safeguarding which is also part of our Policy. Our Child Protection Policy is available to read at any time-please speak to a member of staff should you wish to read it. *

Outings

I agree for my child to go on routine outings with Growing Stars Nursery staff. These outings may include trips to local libraries, parks, and other local places of interest. *

Other Outside Activity

We sometimes organise trips for the children, which take them off the Nursery premises. Staffing levels are always considered and adhered to. We have insurance to cover the children but written permission will be sought detailing the outing venue, date, times, aims of the outing, mode of transport, costs (if any) and the adult/child ratio. A comprehensive risk assessment will be carried out prior to the outing. *

I give consent to my child being photographed at the nursery for training, educational purposes, displays and EyParent reasons. *

Observations

I understand that ongoing observations will be undertaken on my child, to follow and assess their development. These may be in the form of Eylog system,written statements, photographs or DVD recordings. Following the EYFS. *

Sun Protection Cream

I agree for sun protection cream can be applied to the above named child by practitioners at Growing Stars Nursery. *

Medication

If your child is on medication which has to be administered whilst attending nursery, you must complete a medicine consent form giving instructions on times and dosage for each course. Written consent must also be given for the application of creams and lotions. *

Allergies, Asthma and other Medical Conditions

All of the above conditions must be reported to the Nursery Manager, with full information regarding the condition and treatment. *

Change of Circumstances

Please keep us informed if you move house, change your place of work or phone numbers, please ask the office for a change of circumstance form and return it as soon as possible. It is important our records are up to date in case of emergency. *

Discipline

We concentrate on good behaviour. We reward any positive behaviour rather than concentrating on the negative aspects. The Nursery staff are responsible for the behaviour of the children in their rooms. Staff will deal with any misbehaviour and explain to the child why their behaviour is unacceptable (if they are old enough). *

Intimate Care

Intimate care routines are essential throughout the day in order to meet basic needs. This may include nappy changing, supporting children with toileting, changing clothes and giving first aid or medical support where required. *
